You will need at least one lover, who is not a ruler (landed or otherwise), for this to trigger. The Mortal Adoration perk is not necessary but greatly increases the odds.
In 1066 play as Count Otger of Spreewald. He starts with a rival who has high Intrigue and a wife who is his soulmate. Wait for your rival to attempt to murder you and restart until your soulmate saves you.
