The best possible way to get this achievement is to play on the beginner level. Whenever you are doing something with ingredients that does not require the combiner, have two ingredients working on mixing that you know don't go together. Then put them into the reverter while you use the combiner to combine two ingredients that do go together. This way you are progressing in the level while also intentionally messing up things. Rinse and repeat till you have done this 5 times. If you are down to just a few ingredients and have not done it 5 times yet, just start combining an ingredient with water that you know doesn't get combined with water. Once you have done it 5 times combine the last few ingredients together that you need to get your last key and end the level.
Bonus: Combining even two ingredients wrong here will also get you the "Uhmm, Okay?" Achievement. This is also a good one to get the "Whoa, that was close!" achievement by just sitting on the counter with your last Key ingredient next to the cure making machine until you are under 1 minute and then adding it. So you can easily get 3 achievements in one round.
