Unlocked by blowing up The Siren's skull after defeating her.
She's sometimes the boss of Mausoleum 1. To get there, after you beat the boss in The Depths 1 or The Mines 2, a door will appear with a spike on it. Walk into it to sacrifice 2 hearts and it will open the path to Mausoleum.
When she dies, her skin will melt off and her skull will be left on the ground. Blow it up with a bomb to unlock this
