Defeat the Isaac boss as Cain.
The Isaac boss can be encountered by:
1) Beating Mom's Heart 11 times, then beating It Lives
2) Entering an Angel Room after beating Mom's Heart, and walking into the beam of light there
This also unlocks challenge #24 "PAY TO PLAY", if you have the Afterbirth DLC.
