Start up a heart transplant surgery, you firstly need to pick up and remove the cover over the rib cage now move your hand down over the ribs and press 'W, E, R and Space' to leave the pinky finger out over the ribs.

If you're having trouble unlocking this achievement, disable the Anniversary DLC in Steam.
Multiple people have reported unlocking it afterwards.

I was able to unlock it by disabling the DLC and closing every Finger but the Pinky while on the Rip-Cage.
