Near the end of the game, after doing the puzzle with the bells, and destroying the boxes and the coil with the axe whilst avoiding death by electrocution, a long cutscene will run. After it's done, simply press Escape. You'll notice that the "Exit" option now says "Exit the Soulcloud" instead. Choose to exit the soulcloud, and you will receive the alternate ending! And the achievement!
