You need to be in a game with the city state La Vinta.
You need to be the Suzerain, once you are your builders will have the option to build the Colossal Head. Have your builder move next to your holy site and build the Colossal Head.

Have a temple in your holy site.
Jenova_20This wouldn't unlock for me when i built the head before the temple, so it appears to be very particular about the sequence. What I did:
Holy site
Colossal head
Temple
= NO ACHIEVEMENT

Built another head next to the holy site and it unlocked.
