1 1 0*** Spoiler - click to reveal ***From the grave with the ghost you'll want to just continue on past it. You'll come across an area with a lake, and you'll need to inspect the lake. Your character will notice something in it, and you will want to dive in for it. This will cause a giant fish to appear which will eat you, and leave nothing behind except an achievement.
