Worlds of Hurt does not involve any puzzles or anything like that. It's basically 8 challenge rooms where you have to kill all enemies to continue, without dying as there are no saves, and no vita-chambers around.
When you've killed all enemies around, using whatever assortment of weapons, plasmids and tonics you may choose you can save the sister and be victorious.
