In the hallway leading from your bedroom back to the living room (the third room of the game, with the hands coming out of the floor), you'll need to look at a window to collect a piece of broken glass.
Once you've grabbed that, don't back away from the window-keep looking outside of it. After a few seconds, a face will appear, and your achievement will pop once it's faded away.
