Around the 12th chapter or so in the game is one called "Debriefing" which actually has two halves, one with Carla and one with Tyler. This achievement is found in Carla's half and is quite simple. When you try to progress into the dark storage room a mini-game to control her breathing will start. Simply fail the game (takes only a few seconds) and the achievement will unlock.
