The easiest way to get this achievement is to tunnel without the intent to down the survivors. Chase the first survivor you see, using your two abilities to progress their insanity as fast as possible. Once they've reached Tier 3 (you can see the tier increases from the static changing on the survivor icons), leave them alone and find another survivor. Repeat until you've done this to all 4.
Also to note, they don't all need to be at Tier 3 at the same time, as long as they've each been Tier 3 insane at some point in the match it should pop on the last survivor's Tier 3.
