In the Fallen Refuge Map, in the realm of Withered Isle, the generator you need to complete for this achievement is the one in front of the famous DON'T OPEN, DEAD INSIDE DOOR. Complete that gen, escape via the gate once all generators are completed or by hatch and the achievement is yours. You also have to cross your fingers you get the map, due to the changes BHVR implemented with map offerings.
