This can be achieved by getting 5 cactus balls stuck on you at once by walking into them. Cactus balls can be found in the final mountain area of the Mesa biome. Be careful because 5 cactus balls will likely make you pass out even with full stamina, unless you or someone else quickly removes and throws one off of you after getting the achievement.
