Keep all characters alive. In the Star Chamber do not use the knife on the phosphorus round as Rachel. Then make sure Jason does not shoot Rachel but refuse to go into the cocoon. Rachel will then run away and escape.
Later on at the top of the elevator she will attack Nick and Eric if Jason and Salim are alive still. Fail all QTEs and she will kill Eric.
