This is the most easily done with Pyro. In fact it probably can only be done with the Pyro.

Find 2 people on the enemy team. One of those has to be a medic. Light the one that is not the medic on fire and the medic will try and heal that person. Then you ALSO have to light the medic on fire.

The achievement should pop after you have done that.
