You need to fill every clothing slot with items of 100% condition.

You'll encounter most forms of clothing easily throughout your playthrough and several tailoring kits so you can stay indoors and repair all your clothing. The problem is the 2 accessory slots that usually remain vacant.

There are 8 potential items to fill the accessory slots, all having their own troubles of getting;
I've listed them in order of difficulty obtaining. (in my opinion)

Improvised Crampons: 4 scrap, 2 leather, a workbench and 4 in-game hours. You've got yourself some spiky feet.

Improvised Insulation(DLC): Chuck a rock at a bird, get some cloth and a tailor's kit. in 30 in-game minutes you're slightly warmer

Wool Ear Wrap: The last item you'd want to obtain in a container. This item is obtained by random chance in containers or on the floor. This is a exceedingly rare item despite it's lacklustre use and rubbish durability. Just kill some birds and make some improvised insulation.

Ballistic Vest: Found in Blackrock Prison. Approach around the rear of prison, where multiple wolves and a bear can potentially spawn for a rope climb which should lead to a cell block where the ballistic vest.

Moose-hide Satchel:This obviously involves killing a moose, a rare and dangerous animal. You only need to kill one however, for the pelt and the guts. It takes 16 in-game hours to make it, in addition to curing the pelts and guts. The good thing is that the 16 hours doesn't have to be in one sitting.

Foreman's Toolbelt(DLC): Found in the Zone of Contamination, deep in the mines. Find some Chemical Boots and a Respirator and go spelunking.

Security Chief's Rifle Holster(DLC): Not necessarily difficult to obtain, just tedious and time consuming. Found in the Omega Bunker in the Far Territory. You need to fix all of the 6 transmitter towers (1 car battery, 2 wire bundles, 3 replacement fuses, 5 scrap each) found in Lower Great Bear and the forsaken airfield (if starting in Great Bear, do this last). They are located in Mountain Town, Desolation Point, Blackrock, Pleasant Valley, Forlorn Muskeg and of course the forsaken airfield. After that, grab the radio in the Forsaken airfield and find the hidden bunkers in the Far Territory. Following the instructions on-screen you'll eventually find the Omega Bunker. Good luck with that...

Crampons: Found at the end of Ash Canyon in the gold mine, with the maze-like geometry of Ash Canyon and the dangers with that region. I've navigated Black Inlet and Lower Great Bear for a total of 7 hours and can navigate them both better than the 10 hours+ I have in Ash Canyon, If you know your way around the region this shouldn't as bad. Just bring energy drinks and stims.
