This achievement is for beating the glitch level in The End. Glitch levels are unlocked by first beating the boss in the world you are in, and then replaying levels until you see a glitchy bandage girl. Upon completing the level you will unlock the glitch level for that world.

Here is a video showing my strategy for completing the final glitch level:
MegaZeroXHonestly, the world 6 negative level is one of the easiest, strangely enough.
