This achievement unlocks after you've unlocked all other achievements.

This achievement has a higher distribution rate among players than the Masochist achievement, which doesn't make sense as it shouldn't be possible to have this without having all the others as well. However, while you do need the Masochist achievement to unlock the Insanity achievement as well, the Masochist achievement was added later than the rest. So before it was added, people could unlock the Insanity achievement without having unlocked the Masochist achievement, as it simply didn't exist at that point.
W1zZard_Strange that Masochist has 1,9% players who has it and Insanity has 2.3%, shouldn't it be 2,3% for both?

Guyra@W1zZard_: I did explain why in my solution, but I'll try to rephrase it for you:

When Amnesia first got achievements, "Masochist" was not part of the original set of achievements. So many players unlocked "Insanity" without having to unlock "Masochist". In September of 2018 the Masochist achievement was added, and from then on it was required to unlock it if you wanted to also unlock "Insanity".

But that's not how it always was, and that's the reason for more players having unlocked "Insanity"without having unlocked "Masochist". :)
