Best to do this 1v1 with a friend/player.
Have the person you're going to kill use the perk that alows him to run around without a limb. As soon as you start bleeding them out hit X and 2 quick as possible to thrown the pommel if you hit them it will unlock instantly.
Really easy to do and takes less than 30 seconds.
