This achievement is MISSABLE and can only be done in the 8th chapter of the game (Castel del Monte). To unlock the Mistakes Were Made achievement, you need to arrange all Sagittarius discs (blue discs) on the left side and all Capricorn discs (orange discs) on the right side during the Zodiac puzzle (on the astrology counter).
