Unlock all perks, you can do this around when you've got 80-90% tombs and 80-90% fugitive souls. Hidden skills don't count towards this achievement, you can even reset your perks to remove if you put some on hidden and you want to unlock this achievement before 100% the rest of the game, I did, I'm an achievement hunter, so, I needed to unlock this as soon as possible.
