The Achievement description lets you believe you should just pop one blue pill and you get this, but actually you have to kill 5 enemies under its influence in one go. This is pretty easy. I did it on the level "Bozo’s Race Into Space" at one of the power cells. When you destroy the power cell, 5 enemy spawns. I used the rocket launcher to be sure.
