For this achievement, you must kill 4 enemies with a single explosion.
After beating the game, the first place I was able to do this was in the first stage of the "City Center" level. There are 5 enemies grouped up together.
I used the Grenade tactical item with the Frag Grenade attachment. I was also using the "High-Explosive" core item.
