This can very easily be done when you're in the radio hut with Dylan and Ryan. Dylan will get attacked from the ceiling by a werewolf and Ryan will have the option whether or not to cut off Dylan's hand. Choose the option to cut of Dylan's hand and select the chainsaw option to cut off Dylan's hand. Doing this will then unlock the achievement.
