There are two kinds of skulls you can encounter randomly on your travels. The common skull can be picked up and thrown. The rare one is on a pile of other bones. This one can trigger a couple of events. I chose an option of "wink," and succeeded on a stat check. About 40 turns later, the skull returned to kiss me. I then got the achievement.
