I would say this is the hardest of all the achievements, simply because of how annoying gathering every perk is.
Thankfully, you don't need them all to be rank 3 or 4.
Play the game as normal, and get to the end of every "Endless Hordes" room. Each of those has a special set perk (full list here:
https://sundered.fandom.com/wiki/Perks)
Break open all the Corrupted Treasures you can find to get more, and do as many of the purple-covered ones as you can.
I'm not sure if semi-secret Eradicator perk is required, but if so here's how to get it (spoilers, obviously):
*** Spoiler - click to reveal ***In the second area, there's a big drop with a big door at the bottom. It's got 3 or 4 mask statues near it, those are the special secret perks. You find them in each area (2 are in the start), and after collecting them that door will open (I believe it lights up on the map too). Kill the boss in it (it's quite hard, so be prepared), and then you'll get the Eradicator perk.
Overall, this achievement is more about patience than it is skill.
