Note: This is only possible to get during the Prison VHS tape story segment!During the Prison VHS tape story segment, you'll find yourself in a hallway with two rooms on each side. Straight ahead is a sledgehammer.
In each of the four rooms is a brick wall. To progress you must use the sledgehammer to break all four walls(each wall takes three hits), and the sledgehammer should break. However, before you progress further after that, by interacting with the wall where the sledgehammer was, go to the room that contains a wooden crate.
Carry the crate with you to where you can look into the son's room. Place it in front of the hole, interact with the top of the crate to jump onto it, and then go into the room. Open up the closet to find the axe stuck in the wall and a note. Interact with the note, and the achievement will pop!

This is a reference to the Stanley Kubrick directed movie "The Shining", where Jack Nicholson's character, "Jack Torrance", at one point uses an axe to make a hole in a door, peak through, and say, "here's Johnny!" The shape of the axe is also very similar to the one used in the movie.
