They're nine total. Six can be earned over a single playthrough relatively easily, and the other three require significantly more effort. Completing the game on Hard, Infinite Climax, or completing 100 Chapters total on Normal or above. Completing the game on Hard is by far the quickest way to earn this achievement, but completing the prologue over and over again on Normal is by far the easiest. I recommend the former.
