You meet Tyrant once you have all three medallions. You'll see him single handedly move a helicopter and he will continue to stalk you for most of the game. He isn't worth shooting at too often, since he cannot die, but a few bullets in the head/a grenade will stun him temporarily. Speaking of his head, shoot it in order to unlock this achievement, as his hat will come clean off.
