Achievement is called Vague Voices

During chapter 3 a little ways in after you have killed a bunch of zombies you will come to a boiler room with a poison zombie and a combine zombie after you kill them you will notice a ladder in the back of the room you need to use this ladder to get a story item anyway go all the way to the top and you will start hearing some faint voices and the achievement will pop.
AchievementMemesYou can't miss this. You'll have to climb up a ladder to open doors to let a wheel you need drop down. You should hear some voices at the top of the ladder and the achievement will pop.
