You must complete the game without triggering a camera alert. If a camera spots you, that's fine, but don't let an alert happen. You can use the "Fatal Cameras" modifier to give you an immediate game over if an alert happens.
This only applies to alerts caused by cameras. Other alerts will not invalidate the achievement.
