To get this achievement you must defeat Dexter in the desert, then he escapes, later you will get another mission to kill him, just let this pass without doing it.
Now when on the bunker mission, find the bunker and then return to camp, do not go to find the additional bio-weapon, complete the game and this achievement should pop.
