Gamer has been removed
You can get this on a new save, right at the beginning of the game.
Start a new game, and when the skill window comes up, remove the trained points from 'PISTOLS' and then pump them into 'SWIMMING' until it says Master.
Start game, and as soon as you spawn on the dock, the achievement will pop.
Now quit without saving, do the same thing again for the 'MEDICINE' skill!
