After unlocking all challenges you have to complete them as well:

The Freeflow Gold achievement in Batman: Arkham Asylum Game of the Year Edition worth 23 pointsAchieve 24 medals on combat challenges

The Predator Gold achievement in Batman: Arkham Asylum Game of the Year Edition worth 22 pointsAchieve 24 medals on predator

And of course you have to complete the story. You can choose any difficuilty. So you need to complete it at least on "Easy".
