Likely to be acquired on Sky Island; if you kill the large knights with shields on the island and don't unlock the achievement, you missed something earlier in the game.
The most likely culprit: the balls of spikes with faces on them in some of the earlier zones are killable enemies. One can be found on the Whithered Isle if you failed to kill one previously. They can be killed using bombs.
