    Actions
    The mutagen slots are found on the character development page and gradually unlock as you increase in level. To open the final and 4th slot, you'll need to get to level 28. At that point you should be rolling in mutagens (which you get for killing many of the creatures in the game) so should have no problem filling all 4.
