You can get this while going for the Sanitized Earth achievement. You simply have to do a good job of cleaning all of the gunk, gore, viscera, and so on from the level. As with Sanitized Earth, to save yourself from having to re-play the level, you'll want to save before you finish playing so you can load up if you don't get the achievement.
