This can be earned in your first trip to Battery Park.
From the start, if you go straight ahead, then go left when you hit the vertical slabs, you'll find a beggar labeled "Sick Man". He'll ask you to kill him. Grant his wish.
This will not unlock in Hall of Mirrors mode.
