This will require a full playthrough. For this achievement you will need to do everything against the codex (Law/Rules), so make sure your choices are against it. I also purposely failed all cases for this and got trapped in the lab, so make sure the Captain and Michio both die/get shot.
I'm unsure if all of the below will impact the achievement as there is so many options to got through it's hard to tell which ones affect the achievement.
- Give the dog pills at the hospital
- Don't tell on Zena being at Luis bunk
- Get Michio shot
- Get Talya shot
- Fail all cases.
