  You can unlock this form by embracing your urge and killing Isobel as the Dark Urge. You get the option to do this during the Last Light Inn attack. If you didn’t kill her, you can still get it, you just need to do it before you make your way to the Moonrise Towers after you have interacted with the Nightsong.

  You will need to kill Isobel because of The Dark Urge in the second act. The servant will come to you at night and give you the form of the killer.

  As the The Dark Urge, and when you get to the temple of Baal to kill Orin, you will have to engage her in a duel, and after your victory Baal will offer to accept his gift.

  I got mine from the temple, so I am not sure the sequence for Isobel.
