  *** Spoiler - click to reveal ***Early in act 1 there is a group of NPCs (Where the blue dots are in the picture) that are mourning their dying friend talk to them and once the conversation is done talk to the dead body and get the worm from him. After it is in your inventory you can consume it and get the achievement
