This achievement is quite simple, but it requires an upgrade available in the mid-game that allows your arrow to inflict poison. Once you acquire it in the cauldron and equip it, charge the arrow to the maximum, aim at the apple, and then press (F) to shoot a poisoned arrow. You only need to perform this action once.
