I did this one playing as "Hakurei Reimu" (but "Kirisame Marisa" also counts, too) on the song "Disco Night" on Hard (Song Difficulty: 4).The reason why they're both perfect for this achievement because their abilities can keep them hovering in the air without automatically landing.
For the achievement, you need to dodge EVERY enemy in the entire song, but you will need to run through (but don't hit) the Gemini (simultaneous enemies) + Mash enemies.
Since you're dodging the enemies, not grabbing any of the hearts, and playing a Level 4 song, you can earn the "Give Up Treatment"
secret achievement as well!
