Once your Character Creation introduction video concludes, you will enter the game world. You will have a debuff, "Battered and Bruised." While wandering about, some NPCs will comment on your poor disheveled state, and a Wise Destitute Wretch will even suggest that if you hold onto those wounds you may be rewarded... What is that reward, you ask?
If you do not remove this debuff (do not let teahouse keepers heal you) until level 10, head to the Jade Urn map.
There, you will find an RNG-chance Sefra on the western river-front who will provide you a 4 Speed ring. Receiving this item in "trade" is what triggers the achievement.
Note: for roleplay lovers, this achievement will cause you to lose the debuff.
