First you need to reach level 2-8. Once you reach level 2-8 immediately pause the game and select Save & Quit. Now resume your game in level 2-8 and unlock the door and enter the shop. In this shop you need to purchase the hourglass power up. Now exit the shop and pause your game and select back to map.
Now go to the shop in the village and purchase the large wand power up. Now go to the 5th world and reach the level 5-3 shop without dieing and purchase the three remaining power ups here. Exit the shop and your achievement will pop.
