When you enter the basement catacombs,
do not, under any circumstances, punch any door-like things.
Ever. Just focus on traversing the catacombs with the help of a Map.
As you go further in, you will find the medal for the same achievement.
Just be sure to never touch any cell gate you see, there are a lot of them and it's easy to mistakenly punch them while punching the crates that happen to be nearby.
