There are a couple of different ways you can unlock this. Once you've gotten hold of the altered biocell (from the storage room on the 1st floor of your cellblock...the combo can be found in a stuffed animal in the showers, although there are other ways in as well), you need to decide what to do with it.
You can quickly unlock the achievement by using it yourself, although this would invalidate the
if you don't reload after.
Another way to get this achievement is to give the altered biocell to Red Shoes, found in the solitary cell in the yard of the B cell block.
