Complete the Anzio campaign with the used respawn counter at 1 for all missions. You can die, as long as you stay in the same squad. If your squad is low on members and there are friendlies nearby, it may be valuable to teamkill so your character joins a larger squad with more wiggle room. This applies to all "without dying" achievements in ER2.
