To get the Indirect Approach achievement you'll need to kill a combine heavy while their shield is up. Enter a level where there is a combine heavy and before they bring up their shield shoot them once with the shotgun.
Next, wait until they bring up their shield. Once their shield is up you'll need to either get up somewhere high or go around to their back. Have your shotgun ready and shoot them.
